World Spine 2010 Hands On Course (Larissa, Greece)****

Please note that the number of participants are limited.

*Includes nurses, Medical assistants, physician assistants, physiotherapists.

**  Residents/Fellows must attach a letter from the programm director verifying status. Medical Students need to attach a photocopy of Medical Student ID.  (Tier1,2,3)

*** Spouses/Guests cannot earn CME credit nor can they attend Hands on Course.

**** All Participants at the World Spine 2010 Hands On Course have to complete the Workshop Agreement waiver and Release Form  and must send it to T&C all about travel and congress, by one of the following ways:

This Form must be attached to your registration for World Spine 2010 Hands On Course.

The registration fee for participants in Santorini includes:

The registration fee for accompanying persons in Santorini includes:

The registration fee for participants in Larissa includes:

• Participation in all World Spine 2010 Hands On Course scientific sessions and workshops
• Congress bag with the program, (DVD ROM containing Lab manual and syllabus), tourist information and congress related documents
• Opening Ceremony
• Welcome Reception
• Light Lunch
• Coffee breaks
• Access to the exhibition
• Participation in Excursions and social events

Registration

Registration to the World SpineV & World Spine 2010 Hands On Course is made by completing and sending to T&C all about travel and congress, the registration form by one of the following ways:

3. by competing the on line registration form (www.worldspinev.com )

Confirmation

All registrants will receive a confirmation letter.

Payment Conditions

Full payment for the reservation should reach Τ&C no later than 30th April 2010.

Cancellation Policy

Registration

1. Written cancellation received by 31 March. A €50 processing fee will be charged.
2. Written cancellation received by 30 April.  A € 100 processing fee will be charged.
3. Written cancellation received on or after 30 April 2010. No refund

Accompanying Person

1. Written cancellation received by 31 March. A €50 processing fee will be charged.
2. Written cancellation received by 30 April. A € 100 processing fee will be charged.
